Please provide your driving instructor with the following upon first day home pick up of your Behind The Wheel driving lessons :

1. Download and sign our driving school Behind the wheel agreement from the link below 
2. A photo copy of your Learner's Permit.
3. A parent consent specifying you have driven with them on main roads a total of 45hrs or more, 15 of which were after sunset. (A driving log or a signed hand written note will be sufficient).
4. Your Driver Education class completion (Known as Pink Card or Green Card , DEC-1, DTS-B, DTS-36).
5. Payment in form of Cash, Check or Money Order.
